What is ADHD Medication Titration?

Medication titration is the gradual adjustment of a drug’s dose to find the safest and most effective level. It typically starts low and is increased by the clinician based on response and side effects, ensuring safe and appropriate treatment as the body adapts.

Throughout titration, we regularly monitor key health measures including:

Blood pressure
Pulse
Weight

Your ADHD Medication Journey with RTN

Below is a step-by-step overview of what to expect during the ADHD medication titration process at RTN Mental Health.

Reviewing your diagnosis
1

Reviewing your diagnosis

During a 45-minute initial appointment your clinician will review ADHD symptoms, current challenges, physical health, and goals for medication, along with discussing treatment options.
Beginning medication
2

Beginning medication

Following your initial appointment (if there an no medical concerns identified), your clinician will prescribe medication that is most suitable for you. Prescriptions can be sent directly or issued to a pharmacy of your choice.
Clinician guided titration
3

Clinician guided titration

Titration appointments, usually start 4 weeks after starting medication, reviewing health data, side effects, and symptom changes. The clinician may adjust the dose or medication, with follow-ups to find the most effective, stable dose.
Ongoing monitoring
4

Ongoing monitoring

The final titration appointment checks if the medication and dose are stable, effective, and tolerable. If stable, RTN can request a Shared Care Agreement with the GP for ongoing prescribing and monitoring, with annual reviews at RTN; if not, further titration or private prescriptions continue.

How ADHD Medication Works

Stimulant medications work by boosting dopamine and norepinephrine in the brain, improving focus and attention quickly, often within an hour. They’re highly effective but can affect appetite, sleep, and heart rate, and carry some risk of dependence.

Non-stimulant medications work more gradually, taking weeks to reach full effect, and are generally considered safer for people with anxiety, heart conditions, or a history of substance misuse. They tend to be slightly less effective on average but are a good alternative when stimulants aren’t the right fit.

Adult ADHD Assessment FAQs

How long does ADHD medication titration take?
Titration typically takes between 6 to 12 weeks, though it can vary depending on how your body responds. During this time, your prescriber will gradually adjust your dose until the right balance of effectiveness and tolerability is found. It’s a gradual process, and patience is key.
What if medication causes side effects?
Some side effects, such as reduced appetite or mild sleep changes, are common in the early stages and often settle as your body adjusts. If side effects persist or feel unmanageable, your prescriber can adjust your dose or explore an alternative medication. You should never feel like you have to simply put up with them.
Can my GP prescribe ADHD medication?
In most cases, ADHD medication is first prescribed by a specialist. However, once your treatment is stable, a shared care agreement can be arranged, allowing your GP to continue prescribing on an ongoing basis. This process varies depending on your local NHS guidelines.
Do I need regular reviews?
Yes, regular reviews are an important part of managing ADHD medication safely. These check-ins allow your clinician to assess how well the medication is working, monitor any side effects, and make adjustments where needed. Reviews are typically scheduled every few months once your dose is stable.
Can I take ADHD medication alongside other treatments?
Yes, in many cases ADHD medication works well alongside other support such as therapy, coaching, or lifestyle strategies. Combining approaches often leads to the best outcomes. It’s important to inform your prescriber of any other medications or supplements you’re taking to ensure everything is safe together.
Will I need to take medication long term?
Not necessarily, it depends on the individual. Some adults find they benefit from medication indefinitely, while others use it during particularly demanding periods of life. This is something you can review with your clinician over time, and any decision to stop or pause will always be managed safely and gradually.
